WAIVERS OF LIABILITY & HOLD HARMLESS  PHOTOGRAPH/VIDEO RELEASE ASSUMPTION OF THE RISK AND WAIVER OF LIABILITY RELATING TO COVID-19 PARENT CODE OF CONDUCT & PLAYERS ETHICS 2  By signing this document, you agree, warrant, and covenant as follows:   LEAGUE DONATION – I understand that our registration is a donation to the independent, self-funded Maplewood Little Gridders (MLG) and there will be no refunds, under any circumstances, in the event your child quits the program.  I understand that this program is provided by a volunteer organization and requires volunteer hours from parents such as myself throughout the season.  If this MLG continues to struggle for volunteers for activities such as but not limited to coaches, concession stands, field setup, fundraisers, etc. an additional donation fee may be imposed for the 2025-2026 season.  Signups for volunteer opportunities will be provided throughout the season.      RELEASE OF RESPONSIBILITY – I represent myself to be the parent or legal guardian of this player and hereby give my permission and assume full responsibility for my child to participate in MLG and hereby release the MLG, its members, volunteers, and coaches from any and all liabilities whatsoever.  I also understand that NO insurance will be provided.  I will assume all risks and hazards incidental to the conduct of activities of the MLG.  I also understand that the MLG will supply equipment to be used for the season and that I am responsible for returning all equipment in good condition at the end of the season.  I understand that I will be liable to MLG for the replacement cost of any and all equipment that is not returned or that is damaged beyond normal wear and tear.    EMERGENCY MEDICAL AUTHORIZATION – In the event of injury, I, the parent, or legal guardian of the above child do hereby permit the local professional emergency team to tend to my child’s medical needs, which may require transport to a medical facility.  If reasonable attempts to contact the parents or legal guardians or emergency contacts at the listed phone numbers are not successful, I hereby give my consent for an alternate physician or dentist in the event the designated practitioner is not available.    PHOTOGRAPH/VIDEO RELEASE 

I hereby grant MLG permission to display and share electronically via the Internet (Facebook) and/or in other displays and/or in the news media, photography, and or video footage of the participant in MLG activities.  ASSUMPTION OF THE RISK AND WAIVER OF LIABILITY RELATING TO COVID-19 By signing this document, you acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that the participant may be exposed to or infected by COVID-19 at MLG events.  Parent Code of Conduct & Player’s Ethics: I certify as a parent/guardian of a child participating within MLG that I/We have read & received a copy of the Parent Code of Conduct and agree to abide by the terms and conditions and that my child has read & received a copy of the Players’ Ethics and agrees to abide by the terms and conditions.  Failure to abide by the Parents Code of Conduct for any family members may result in the family member’s dismissal from MLG activities.  Failure to abide by said dismissal will result in the player being removed from the team.  All such actions will be determined by current the MLG Board members.  This authorization expires on December 30th of the registration year.
